My family and I are looking around for an English Mastiff puppy and I have a question about one we came across. I saw a for sale add for a female fawn which is exactly what we are looking for. It is from a good breeder from what I can tell and has excellent lineage...and that's where the problem starts. It is from brother/sister parents. The seller is wanting to sell it without papers and with a spay/neuter contract for obvious reasons but are also giving a 2 year genetic health guarantee.

I don't know how I feel about this...she will be a family pet and would be spayed anyways since we are not interested in breeding her and no papers would not be a huge issue but honestly, what health or other issues could I run into with an incest puppy?

I agree with Smart. I know accidents happen but IMO I feel as if a reputable breeder would absolutely not allow such an accidental breeding to occur. Unless they purposely bred brother/sister?

A reputable breeder would have no reason not to sell with papers. If the breeder is selling puppies without papers that means they likely bought those dogs from a breeder (who they lied to) on a pet contract and are breeding them to make money not better the breed. Does this breeder health test (OFA, CERT etc)?

Sorry, but everything you posted are huge red flags that this is NOT a good breeder. Also remember, a health guarantee is only as good as the person who stands behind it. If this breeder is breeding dogs without papers, does not health test, did not buy the dogs to breed (defrauded the original seller of the puppies) do you have confidence they will actually stand behind their health Guarantee? At the end of the day you will have a puppy with no guarantee and likely no breeder support.
