Thyroid Issue?

Hello, fellow mastiff parents!

I know it's been a long time since I've been on here, but life has been crazy. I wanted to get everyone's opinion on something with Axel.

Axel is 9 and a half. For those who don't know me on here, he is a blue Cane Corso that weighs in at about 134. About 5 years ago, Axel started to gain weight just in his belly. I cut his food back and changed it, and he did lose a little weight, but not much. The dog always acts like he is starving. A few months ago, I noticed a good sized mass on one side of his neck. After using Dr. Google, I thought it might be a goiter and he had a thyroid issue. That would have explained a lot! I took him in, and his level was at 1.8 (which is on the low end of the normal range of 1-4). His vet said it's probably a fatty tumor, but he was surprised that his thyroid was normal. He offered to put him on the medication to see if it helped him, but I declined. This morning, I noticed he had a few patches on his sides where he is losing fur. So, now I'm left wondering, does he really have a thyroid issue? Then, I remembered Axel had ate dinner about 30 minutes before I took him, so I'm wondering if he should have been fasting. I'm attaching a picture of one of the patches.

Does anyone have any advice, or maybe you went through something similar? Should I get it rechecked? Should I just put him on the meds? SOS!
